Navigate to the Equipment and Facilities Service Request form on the Department of Biochemistry and Biophysics’ Intranet.  (If this link is broken, navigate to UNC’s Department of Biochemistry and Biophysics home page, scroll to the bottom and look for the link to the department intranet.) Enter your Onyen and password.

Once logged in, click on the Equipment and Facilities Service Request form. Note that there is a great range of possible services. They include (1) replenishment of film processor or dishwasher fluids, (2) facilities repair, e.g., of lights that are out or sinks that are leaking, and (3) departmental equipment repair. For departmental (core) equipment repairs use the dropdown menu to identify a specific piece of equipment. Please leave detailed information about the nature of the malfunction in the comments box.

Often when the office receives repair requests, they send an acknowledgement email to the person who made the request. If a work order number is generated, save it to refer to in any future communications with the office. If your request is something that might be requested by someone else, please notify the relevant parties so that the department does not receive multiple requests regarding the same issue. If the issue is with an autoclave or a dishwasher, record your service repair request on the whiteboard next to it, as well as any other information that would be helpful to other users, such as the nature of the problem you experienced.
